We analyze spatial patterns of links between high and low temperature extremes and cardiovascular mortality and morbidity (hospital admissions) in the population of the Czech Republic over 1994-2013. Geostatistical analysis of the Census data (spatial cluster analysis, geographically weighted regression) is used to identify regions with relatively high and low socioeconomic status (SES) and those with predominantly industrial/urban and agricultural/rural environment. Spatio-temporal Generalized Additive Models (GAMs) are applied to model relationships between ambient temperature and mortality or morbidity in the identified regions. Environmental (land cover) and socioeconomic (age, SES, population density) confounders and modifiers affecting the temperature-mortality/morbidity relationships are identified. After taking into account lagged effects, we find that heat-related mortality is most pronounced in highly urbanised districts with low SES, while cold stress leads to excess mortality especially in rural districts with low SES. The results help identify high-risk population groups and regions with the strongest adverse effects of heat and cold periods due to differences in social deprivation and environmental conditions in the Czech Republic.
