The review outlines basic knowledge of the molecular structure, sources of tissue diversity, and modulation of BKCa channels by endogenous and pharmacological agents. Particular attention is paid to the heart with an emphasis on the role mitochondrial BKCa channels in various forms of cardioprotection against acute ischemia/reperfusion injury
