Kapitola předkládá základní statistické principy, které souvisí s plánováním a realizaci klinických studií. Pojednává podrobněji o smyslu a realizaci takových principů, jakými jsou např. randomizace, volba kontrolní skupiny, zaslepení klinické studie, zobecnitelnost závěrů klinické studie, jakož i o etickém rozměru těchto studií, zejména o požadavku na rovnováhu mezi užitkem aktuálně léčeného pacienta a pacienta prospektivního, který může těžit z výsledků ověřování medicínských hypotéz a poznatků získaných realizací klinické studie. Kapitola se též věnuje popisu jednotlivých fází klinických studií a jejich historickému vývoji.

The chapter discusses basic statistical principles underlining the planning and realisation phases of clinical trials. It provides a practical insight into the purpose and a way of materialising such principles as are the randomisation, choice of a control group, blinding of a clinical trial, generalisation of the results obtained from a clinical trial, and also into ethical extent of such studies. We discuss the requirement of balancing the utility of the study between current and prospective patients, the latter likely profiting from the findings related to testing medical hypothesis and the clinical trial realisation. The chapter also describes individual phases of clinical trials and gives a historic perspective of their development.
